Account verification
Overview
Verification forms are used when an Account Owner and/or listed Contact is unable to log into their AccountCenter, and do not have access to the primary email address on file for their account. Whenever a representative is unable to authenticate access to an account, they can reset the password by resending the AccountCenter Welcome Letter.
Instructions
You can reset your password at the AccountCenter login page, by requesting a link be sent to the email address on file for your account. However, if your email address is invalid or inactive, you will need to complete a Verification form. The forms are located on our website, https://mediatemple.net/company/legal under Important Documents.
Account Owner Verification Form
The Account Owner Verification form requires the listed Owner to provide the following:
- Domain Name or Account Number.
- First and Last name, as listed on the account.
- New primary email address.
- Copy of a government issued identification. Passport, Photo ID or Drivers License etc.
- Signature and Date
Account Contact Verification Form
The Account Contact Verification form requires the listed Contact to provide the following:
- Domain Name or Account Number
- First and Last name, as listed on the account.
- Type of Contact (Admin, Tech, or Billing).
- New primary email address.
- Copy of a government issued identification: Passport, photo ID or driver's license etc.
- Signature and date
The above forms, including the photo ID, can be sent via fax to (310)388-3244. Once the form has been completed, you will receive an email at the email address you provided on the Verification form. This email will contain a link that will allow you to reset your password for the AccountCenter.
